| All participants, Team Members, Me Too!, Seniors Too! and Kids Too! are required to complete a health assessment before and after the 8-week challenge. Health assessments can be completed by your own doctor or you may qualify to use a Ho'ola Lahui Hawai'i/Kaua'i Community Health Center Clinic. (If you have had any of these health tests since May 2004, they will be accepted with your doctor's signature and approval).
Health assessment includes:
- Medical clearance from your doctor or from Ho'ola Lahui Hawai'i/Kauai Community Health Center (medical clearance forms will be available at registration).
- Cholesterol Profile
- Body Mass Index (Height/Weight)
- Blood Pressure
- Health Questionnaire
- Blood Sugar
- HbA1c (for people with diabetes only)
After registering, and if you qualify, you will receive a voucher which will allow you to make an appointment at a Ho'ola Lahui Hawai'i/Kaua'i Community Health Center Clinic.
20 Fit Bucks will be given to each team member who completes the required health assessment by February 26th.
