Kauai's Great Weigh Out

Kauai's Great Weigh Out is an 8 week health and fitness challenge. The objective is to shed unwanted pounds and become more physically active through a sound nutrition and exercise program. Here's how it works:

1. Fifteen teams of ten people each, will be forming island-wide.
2. Prizes will be given to the 2 top teams with the greatest percentage weight loss and the 2 top teams that earn the most Fit Bucks. A team may only win one prize. In the event a team wins both categories, they will be eliminated from one of the categories.
3. Registration packets are available for pick up at Ho'ola Lahui, Department of Health or Lihue, Waimea, Kapa'a, Koloa, Kalaheo or Kilauea Neighborhood Centers beginning November 3, 2003.
4. Registration opens on Monday, November 17, 2003 at 9:00 am. by faxing completed form to 823-4100. Any registration submitted before 9:00 am. will not be accepted.
5. The first 15 teams to fax COMPLETED registration form and team application will be notified by phone or email. Fax completed registration forms to: Colleen McCracken at Samuel Mahelona Hosptial: 823-4100 Beginning November 17, 2003 at 9:00 am.

Team Captain Commitments

1. Recruit a team of 10 committed people (including you) who want to improve their lives through a sound nutrition and exercise program.
2. Choose people willing to participate in Weigh Out daily and weekly activities who will be committed to submit their weekly log book information to you by the weekly deadline!
3. Recruit a team member to be Co-Captain. We require that both captain and co-captain list a current email address.
4. COMPLETE the registration form with all names and signatures. Incomplete registration forms will jeopardize your acceptance as a team.
5. Team Captains are required to attend the Captain, Coach Luncheon Meeting on Saturday, January 3, 2004. Time and location will be announced.
6. Collect $5.00 from each team member and bring it to the Luncheon Meeting on Saturday, January 3. (Cash only please)
7. Meet with your team after the Captain's Luncheon and before the Kick-Off on January 10th to go over the entire program.
8. Collect team member activity and nutrition log Fit Buck totals earned weekly and submit to Jim and Pat Kerber, the Fit Buck coordinators, before the established weekly deadline!
9. Stay in touch with your team to provide encouragement and moral support. Weekly team meetings are highly recommended!
10. Remember, Weigh Out Captains are HEROES!

Team Members Commitment

1. A team member must be willing to join a team with 9 others who commit to 8 weeks of exercise, healthy eating and education through the Kauai's Great Weigh Out program. (January 10 through March 6, 2004)
2. The team's goal is to practice healthy living and win prizes for team weight loss or total earned fit bucks. Team member's individual efforts help the team succeed.
3. This effort includes:
a. Logging personal daily exercise and servings of fruits and vegetables.
b. Submitting weekly log totals to team captain on time.
c. Participating in team meetings called by your captain.
d. Earning extra fit bucks by attending as many KGWO sponsored activities as possible.
4. Activities include but are not limited to:
a. Daily walks and exercise classes scheduled in different areas of the island.
b. Wednesday evening educational classes at Wilcox Hospital.
c. "Weigh In" Saturday January 10, "Mid Weigh" Saturday February 14 and "Weigh Out" Saturday, March 6.
5. You agree to complete and return a questionnaire 6 months after the end of the 2004 program.
